Clay County Social Services offers MnCHOICES assessments to determine eligibility for services and administers the Elderly Waiver, Alternative Care and Essential Community Supports programs. These programs authorize long term services and supports to meet the needs of individuals 65+ who are living in the community. These programs are administered by counties and tribal health agencies.
Alternative Care (AC) Program provides home- and community-based services to people age 65 and older who meet NF (Nursing Facility) LOC (Level Of Care) and are not eligible for Medicaid. This program is intended to prevent and delay transitions to nursing homes. This program shares the cost of care with clients by maximizing use of their own resources.
Essential Community Supports provides supports to people age 65 and older who do not meet NF (Nursing Facility) LOC (Level Of Care) and are not eligible for Medicaid. This program is intended to meet emerging needs for community support.
Elderly Waiver (EW) for people over the age of 65 years who require the level of care provided in a nursing facility and choose to reside in the community (DHS Aging and Adult Services Division administers the EW program).
