Renewal Forms

All providers must renew participation in the Child Care Assistance Program (CCAP) every 2 years. To renew your CCAP registration you must complete the forms below and return them to the CCAP Provider Payment Contact. The renewal forms must be submitted to Clay County Social Services prior to the provider's renewal date:

Submit a copy of your license or self-declaration certificate along with the above forms.
